Abstract: In August 1993, a Phase II trial of taxol given as a 3-hr infusion in relapsed NHL was initiated. Patients were eligible if they had pathologically documented relapsed low grade or intermediate grade lymphomas, a performance status of greater than or equal to 60% (Karnofsky), measurable disease, a platelet count of greater than or equal to 100,000/mm3 and an absolute neutrophil count of greater than or equal to 1000/mm3. Patients were excluded if they had HIV or CNS lymphoma, a history of arrhythmia, or active infection. After pre-medication with Decadron, Benadryl, and cimetidine, all patients received a starting dose of 200 mg/m2 of taxol infused over 3 hr in an outpatient setting. If no significant neutropenia (less than 500) or thrombocytopenia (less than 100,000) occurred the dose was escalated to 225 mg/m2 and 250 mg/m2 in the subsequent cycles. So far, 30 patients have been registered, of whom 19 are evaluable for response after 2 courses. Data are shown in a table. The 2 patients with diffuse large cell lymphoma who achieved CR with taxol received 2 prior chemotherapeutic regimens each. The treatment was very well tolerated and no patient experienced anaphylactic reaction or cardiac arrhythmias. The median neutrophil count nadir was 800/mm3 and the median platelet count nadir was 144,000/mm3. Our preliminary results indicate that taxol given at this dose and schedule is safe and active in relapsed NHL.
