Radiotherapy of primary CNS lymphoma in HIV-positive and negative patients: ten years of treatment experience at UCSF (Meeting abstract).

Abstract:
In a ten year retrospective review, charts of all patients treated at UCSF in the Radiation Oncology Department for primary CNS lymphoma between January 1982 and January 1992 were analyzed. In total there were fifty cases, 35 were known to be HIV+, the remaining 15 were felt to be classical nonepidemic CNS lymphoma. The median length of survival for HIV+ patients was 2 months as compared to 11 months for HIV- patients; although, the average KPS score at presentation for HIV+ patients was 50 while that of HIV- patients was 30. The average age of the HIV+ patients was 37.5 years, while that of the HIV- patients was 50.8 years. Five of the fifteen HIV- patients were female, all of the HIV+ patients were male. Forty-two patients were treated in the years 1987-1992, representing a more than five-fold increase in the incidence of primary CNS lymphoma as compared with the incidence in the preceding five year period. This increase was primarily attributed to the immunosuppression associated with the AIDS epidemic. The most commonly administered total radiotherapy dose was 40-41 Gy, with a range of 300 cGy to 72.5 Gy. We currently recommend HIV+ patients receive 4000 cGy whole-brain in 3 weeks, while HIV- patients receive 6000 cGy in 6 weeks plus multidrug chemotherapy.
