Isolation of Mycoplasma species from bronchoalveolar lavage of HIV and non-HIV patients.

Abstract:
From March, 1991 through March, 1992, 94 consecutive bronchoalveolar lavages (BAL) performed for diagnosis of suspected infectious pulmonary disease were cultured for mycoplasma as part of the routine microbiological workup. Polymerase chain reaction (PCR) specific for Mycoplasma fermentans was also performed on 89 of the BAL samples. Of the 77 patients who underwent the procedure 39 were seropositive for HIV and 38 were HIV seronegative. Mycoplasma isolates were identified by metabolic inhibition with species specific antisera. Mycoplasmas were isolated from 11 of the HIV positive patients: Mycoplasma pneumoniae from 1, Mycoplasma hominis from 4, and Mycoplasma salivarium from 7. From 4 HIV negative patients Mycoplasma hominis (1) and Mycoplasma salivarium (3) were isolated. Culture and PCR for Mycoplasma fermentans were negative for both groups of study patients. Ten of the 11 mycoplasma positive HIV positive patients were coinfected with other potential pulmonary pathogens. No specific pattern of association of Mycoplasma species with other pathogens was observed. Mycoplasmas were isolated more often from HIV infected patients than from patients with other types of chronic disease in our medical center (p = 0.095 by Chi square test with Yates correction). Mycoplasma pneumoniae was an infrequent cause of pulmonary disease in these groups. The role of M. hominis and M. salivarium was unclear. Mycoplasma fermentans was not identified in either the HIV positive or HIV negative chronically ill patients studied by culture or direct PCR.
