Mycobacterium tuberculosis bacteremia at Cook County Hospital: a 30-month experience.

Abstract: Patients with Mycobacterium tuberculosis (MTb) bacteremia are not well described. Over a 30-month period, Cook County Hospital had 497 patients with documented MTb, 68 of these had mycobacterial blood cultures done, 14 were positive. MTb constituted 21% of all mycobacterial bacteremias. All 14 were HIV-positive with an average CD4-lymphocyte count of 36; 8 had previous AIDS-defining infection. Identification required an average of 53 days. 8 had MTb also identified from sputum, 7 from an extra-pulmonary site. 2 had MTb isolated only from blood. Treatment was started an average of 10 days after admission. MTb bacteremia was not a factor in initiating treatment. 11 had clinical improvement, 3 died. MTb comprises a significant proportion of mycobacteremias in late-stage HIV disease. The delay in identification limits its clinical usefulness, however it is a non-invasive test and in some patients may be the only way to confirm the diagnosis of MTb.
