Abstract: Viremia with HIV can provide insights into the progression of HIV infection. Our HIV culture laboratory supports an AIDS Clinical Trials Unit (ACTU) but also processes other non-ACTU samples. We questioned if the rates of positive cultures differed between these two groups. Cultures were performed using 6 x 10(6) patient cells cocultured with 6 x 10(6) phytohemagglutinin-stimulated non-HIV infected donor peripheral blood mononuclear cells. Cultures were sampled for p24 antigen twice weekly. The HIV coculture was considered to be positive when two or more HIV p24 antigen determinations were > or = 30 pg/ml. For 135 adult ACTU patients cultured between 1988 clinical and 1992 the positivity rate was 67%. Pediatric patients participating in ACTU protocol 138 and protocol 128 (44 cultures for each protocol) showed a positivity rate of 46% and 77% respectively. For 69 (60 pediatric) non-ACTU patients the positivity rate of 15% was significantly different from adult and pediatric clinical trials groups (p < 0.001). We conclude that in these patient populations the inclusion in an ACTU study was associated with higher rates of HIV-positive cultures.
