Abstract: HIV p24 antigenemia is a surrogate marker for assessing therapeutic efficacy in clinical trials and monitoring patients on antiretroviral therapy. Recently, measurement of HIV p24 antigenemia after dissociation of immune complexes (IC) has been shown to increase the sensitivity of detection of p24 antigenemia. This study compared IC dissociated and non-dissociated HIV p24 antigen detection in a longitudinal study of 26 HIV infected patients just prior to, and at 1, 2, 4, and 8 weeks after the initiation of ZDV therapy. Plasma HIV p24 antigenemia was estimated using the Coulter assay, and the Coulter IC dissociation assay. Plasma was stored frozen and assays were done for all time points of an individual patient at the same time. The non-IC dissociated assay detected p24 antigen in 6 of 26 patients (23%), the amount of p24 antigen decreased in all 6 patients during therapy, and the decrease occurred by the first week of ZDV therapy. The IC dissociated assay detected p24 antigen in 18 of the 26 patients (69%), the amount of p24 antigen decreased in 17 of these patients during therapy, and a decrease occurred by the first week of ZDV therapy. Of the patients positive by the non-dissociated assay, all were also positive by the ICD HIV p24 assay. The average CD4 count was 226 per microliters (range 5-708 per microliters), and there was no significant correlation between CD4 count and ICD p24. We conclude that IC dissociation assays are a more sensitive method to monitor changes in p24 antigenemia in the period immediately after initiation of ZDV therapy. However, it is important to do the IC dissociated HIV p24 antigen assays from an individual patient at the same time.
