Results of chemo-radiation treatment for epidermoid cancer of the anal canal (Meeting abstract).

Abstract: The medical records of 42 patients (pts) with a diagnosis of anal cancer treated under a standardized chemo-radiation protocol were reviewed. Chemotherapy consisted of 5-FU and mitomycin C. Radiation, concurrent with the chemotherapy for the first 4 days, consisted of 30 Gy in 15 fractions. 27 females and 15 males were treated. Five male pts were HIV-positive. Perianal cancers were excluded. Median follow-up time is 31 mo. All pts completed the treatment protocol. 12 pts (29%) had minor protocol violation. Six pts (14%) were T1 at presentation, 23 (54%) were T2, 8 (19%) were T3 and 5 (12%) were T4. Currently, 36 pts (86%) are alive without evidence of disease ( NED). Ten pts have recurred, 9 only locally (LR). Mean time to LR was 21 mo (range 2 to 52 mo). Two pts (5%) have died of intercurrent disease and 2 pts have died of anal cancer. For T1 pts, 100% are NED, LR was 17%. For T2 pts, 87% are NED, LR was 4%. For T3 pts, 75% are NED, LR was 38% and 2 pts died of their disease. For T4 pts, 60% are NED, LR was 80%. Salvage abdominoperineal resection (APR) was performed on all 9 LR. Of these 9 pts, 7 (78%) are NED with a mean follow-up of 23 mo (range 2-54 mo). There was a trend towards increased LR for female pts (33% vs 13%). Both LR and survival correlated significantly with size of tumor at presentation. APR, at present follow-up, appears to be effective salvage therapy.
