Abstract: We have used monoclonal antibodies in 2-color flow cytometric analysis to study the phenotype of peripheral blood lymphocytes (n=29) and tumor-infiltrating lymphocytes (n=11) of untreated Saudi patients (pts) with nasopharyngeal carcinomas (NPC). Normal healthy Saudis were used as controls. We found the percentage of B lymphocytes (CD19, 8%) and the T-helper/T-suppressor cell ratio (CD4/CD8, 0.8) were significantly lower, while the percentage of T-suppressor cell (CD8, 46%) was significantly higher in NPC pts. We also found pts displaying activated phenotype with a significantly higher expression of HLA-DR+ (18%) antigen among the circulating T lymphocytes. When we looked at the T/L suspensions, the majority were mature T (CD3) lymphocytes (61-93%) with a variable of T-cell subsets: T-helper/inducer cells (CD4, 28-56%) and suppressor/cytotoxic cells (CD8, 22-62%). The ratios of CD4/CD8 were 0.5 to 2.5, however, in 5 cases CD4-positive cells were predominant, while CD8-positive cells dominated in 3 other cases. The T/L also showed a variable percentage of NK cells (CD16, 2-23%). Among the CD3-positive cells, we found the CD expression of HLA-DR+ antigen (14-90%), which indicated a phenotype of activated T cells. No statistical significance between the various lymphocyte subsets and the pts' clinical stage was found. The relationship between lymphocyte subsets, pts' clinical outcome and the prognostic significance of phenotypic patterns of tumor-infiltrating lymphocyte is currently under investigation.
