Abstract: The development of NHL in AIDS conveys a poor prognosis with less than 10% of patients (pts) surviving beyond 2 yr. However, these aggressive tumors often require systemic chemotherapy to bring about effective palliation. In the last year, we have investigated the combination of cyclophosphamide 750 mg/m2, epirubicin 50 mg/m2, vincristine (Oncovin) 2 mg and prednisone 100 mg/day x 5 (CEOP), given at 50-100% dose schedule every 3-4 wk. To date, 17 pts have been treated (16 male, 1 female), age range 27-79 yr (median 35 yr), 7 with immunoblastic, 5 with diffuse large cell, 2 with Burkitt's and 3 undifferentiated lymphomas. Of 15 pts evaluable for response (2 pts too early to assess), 6 pts (40%) achieved a complete remission with 5 (33%) achieving a partial remission for an overall response rate of 73%. Four of 11 evaluable pts remain alive beyond 6 mo (36%). Interestingly, survival and response have correlated with presenting CD4 count or dose intensity. Generally, therapy has been well tolerated with few pts developing greater than grade II WHO toxicity. Preliminary data indicate this to be an effective and tolerable regimen that compares favorably with previously described approaches.
