Abstract: A major expansion of the CD8+57+ peripheral blood T lymphocytes (ranges 15-60% PBL; normal values: 7 +/- 5%) is frequent after transplantation or viral infection. We observed oligoclonal TCR rearrangements in CD8+57+ T cells from 8/9 BMT leukemia-free patients (pts), but not in the control, autologous CD57- T cells. Thus, we addressed the question of the TCR variability in the CD8+57+ PBL expanded after BMT or HIV infection. We performed a PCR analysis of the VB segments' usage in the CD8+57+ PBL purified from 9 BMT and 4 HIV+ pts with a series of 20 oligonucleotides specific for the VB families. A marked restriction of the VB repertoire was observed in these cells, with a predominant usage of the VB 16, 17 families. Data were confirmed by direct sequencing of the PCR products. In contrast, the complete set of the 20 VB families tested were expressed in HIV+ pts. The CD8+57+ T-cell expansion may thus result from the growth of a limited number of T-cell clones, with a marked restriction of VB gene usage, due to antigen/superantigen selection or T-cell differentiation abnormalities occurring after BMT but not during HIV infection.
