Radiotherapy (RT) for bronchopulmonary AIDS-related Kaposi's sarcoma (KS) (Meeting abstract).

Abstract:
The aim of this prospective study was to evaluate the efficacy and tolerance of thoracic RT for bronchopulmonary KS, in patients (pts) who never received chemotherapy. Between September 1990 and September 1992, 17 male pts with lung biopsy-proven KS, and/or positive bronchoscopy findings were included. Ten had experienced an opportunistic infection within a mean of 18 mo prior to entry. The mean CD4+ count: 41/mm3 (14-116). The condition of 4 pts worsened rapidly and they were unable to receive RT. The treatment schedule was 10 Gy in 10 fractions over 2 wk (anterior and posterior fields). For cutaneous KS, 11 pts had received RT, and 4 pts interferon prior to treatment. Antipneumocystis prophylaxis was used in all pts. Improvement of dyspnea was noted in 7/13 cases 1 mo after the start of treatment. Radiologic improvement was obvious in 2 cases. The cause of death of 10 pts was respiratory failure related to KS and the median survival time (actuarial method) was 5.6 mo. We conclude that RT is well tolerated. It is useful as a palliative treatment to relieve dyspnea, but the duration of improvement is short, and the objective response is rare.
