Abstract:
We sought to determine if women with AIDS have rates of invasive cervical cancer higher than women in the general population and to determine if this risk increases as immunodeficiency progresses. We linked AIDS and cancer registries in 7 state and local health departments, computed age-, sex- and race-adjusted numbers of expected cases for time periods relative to AIDS diagnosis, and compared observed linkages to expected cases. 15 women had AIDS and invasive cervical cancer. Their median age was 34 yr, 60% were black, and HIV transmission category was not different from women with AIDS without cervical cancer. Comparison of observed to expected cases by time before or after AIDS is presented in a table. The risk of cervical cancer was elevated but stable throughout the time period before and after AIDS onset. These results did not change when observations were adjusted for potential pre-AIDS cancer-related mortality. While women with AIDS are at increased risk for invasive cervical cancer, their risk did not increase further with extremely severe immune deficiency as manifest by AIDS. Without doubt, some of the elevated risk of cervical cancer is due to life-style factors that are common between cervical cancer and HIV risk. Further studies are needed to determine if HIV has a role in these modest, stable increases.
