Abstract: Development and progression of Kaposi's sarcoma after institution of glucocorticoid therapy has been reported in patients (pts) with HIV infection and in renal transplant recipients. We examined the localization and level of GR in 'normals' and in 25 pts with HIV using sensitive immunocytochemistry and RIA procedures (Antakly, Cancer Res, 1989; ibid Endo, 1990). GR mRNA levels were monitored by PCR. All tumors express high levels of GR which was demonstrated in 'spindle cells' as well as endothelial and epidermal cells. The cytoplasm as well as nucleus contain the GR. As compared with normal heterosexual men and women without HIV, KS tissues contain more than twice the GR level of normal skin. Also, GR levels of the KS lesion were significantly higher than uninvolved skin from the same individuals. Next, we studied whether several cytokines that regulate KS cell growth also upregulate cellular GR levels. Oncostatin M, IL6 and TNF alpha resulted in an upregulation of GR expression. These data thus demonstrate the localization and difference in GR expression between 'normal' and KS tissues and show that growth promotion by cytokines may be potentiated by an induction of cellular GR.
