Immunological events following chemoimmunotherapy combination of metastatic malignant melanoma (MMM) (Meeting abstract).

Abstract: 16 patients (pts) with MMM were included in this study. They received 100 mg/m2 of cisplatin (CDDP) over 4 hr at days 1 and 28, followed by 18 MIU/m2/day of interleukin-2 (IL2-Eurocetus) in continuous infusion from days 3 to 6, 17 to 21, 31 to 34 and 45 to 49. Alpha interferon (IFN-Roche) 9 MU/day was given sc 3x/wk. Immunological parameters were studied on days 3, 5, 7, 17, 19, 21, 31, 33, 35, 45, 47 and 49. At each IL2 infusion, a severe lymphocytopenia was shown, but 2-3 days after discontinuation of IL2, a lymphocytosis was detected, which was always higher than the pretreatment level. In all cases, a correlation between lymphocytes expressing IL2 receptor and soluble IL2 receptor (sIL2R) level was found. We found a significant change of sIL2R levels between responder (R; n=10) and nonresponder (NR; n=6) pts during the first two courses (p less than 0.005), but no difference from the third course, suggesting that the cells from NR may be slower in becoming stimulated. We also observed a significant correlation (p less than 0.02) in lymphocytes expressing NKH1+ CD16+ between R and NR, only during the third course. Furthermore, no significant change in CD4/CD8 ratio, both at onset and on treatment, was observed between R and NR pts. NK activity was not affected by CDDP treatment, but a significant difference could be shown (p less than 0.05) between healthy volunteers and MMM pts before treatment.
