Abstract: Hemopoietic growth factors have recently been incorporated in a number of clinical trials both in hematology and in oncology, and results (although preliminary) are certainly promising. Correction of anemia by the administration of rEPO has been achieved in anemia of renal failure and responses have been observed in a number of other anemias (such as that of AIDS, rheumatoid arthritis, myeloma, cancer and pure red cell aplasia). G-CSF has shown significant activity in raising the circulating neutrophil counts in myelodysplastic syndromes (MDS), congenital neutropenic states, idiopathic neutropenia, cyclic neutropenia and neutropenia associated with hairy-cell leukemia. GM-CSF, a cytokine that stimulates the growth of granulocytes and macrophages, is under intensive study in a large number of clinical trials. Emphasis is placed on treatment of MDS, in combination with rEPO, to accelerate hematopoietic recovery following chemotherapy (CT) and enhance peripheral blood stem cell (PBSC) collection for autologous reconstitution. In addition, it is being incorporated in the treatment of AML either immediately after CT, to decrease the duration of neutropenia, or (more importantly) before CT, in an attempt to increase neoplastic cell kill. Responses with GM-CSF have also been observed in severe aplastic anemia and Fanconi's anemia. IL-3 or multi-CSF is also under study alone or in combination with other cytokines for the treatment of MDS and the Diamond Blackfan Syndrome, for CT-induced neutropenias and for PBSC collection. Finally, the recently identified stem cell factor (c-kit ligand) has not yet entered clinical studies, but animal studies show great promise.
