Polychemotherapy AVB schema in Kaposi's sarcoma in HIV-infected patients (Meeting abstract).

Abstract:
Chemotherapy was planned on those HIV+ patients (pts) with Kaposi's sarcoma (KS) with unfavorable prognostic factors. In a first group which showed rapid progression with CD4 A and with skin affects, chemotherapy was carried out with vincristine/vinblastine (VBL). In a second group with III/IV stages, CD4 200 and prior opportunistic infections polychemotherapy AVB schema (Adriamycin [ADR], vincristine [VCR], bleomycin [Bleo]) was performed. Materials and methods: 16 pts with KS were studied; they had an av age of 35 yr (26-44). A total of 10 pts showed Stage II (cutaneous and mucocutaneous localizations), and 6 were Stages III-IV metastatic localizations in the lungs and digestive systems. Of the 16 pts, 12 received the AVB schema (ADR 20 mg/m x 1, VCR 2 mg/m x 1 and Bleo 10 mg/m x 1 at 14-day intervals), and 4 received the alternating VCR/VBL schema. The av cycle per pt was 5. Results: 10 pts were evaluated with the AVB schema. In 6 of them (60%), a partial response in skin lesions was obtained with depigmentation and a lessening of the lesions, with an av duration of approx 3 mo. The most significant collateral results were severe pancytopenia in 4 pts and an important number of accumulated infections (6 with pneumonia, cerebral toxoplasmosis 1, pulmonary TB, herpes zoster 2, and Candida, 4). Conclusions: Although the number of pts is not very important, we think that chemotherapy provides an objective improvement in 50% of the pts within a very short period of time, and that these kinds of pts show a high number of accumulated infectious processes that impede the carrying out of adequate chemotherapy.
