Abstract:
We retrospectively reviewed our experience with nutritional support of malnourished AIDS patients (pts) between 1987 and 1989 to determine the cost of therapy and the objective changes in serum total protein and albumin and nitrogen balance. Ten homosexual male AIDS pts received aggressive nutritional support. The av age was 39.8 yr. One pt was CDC class IV:A and 9 pts were CDC class IV:C-1. The av wt loss was 11.3 +/- 6.2% SD. All 10 pts were unable to tolerate oral alimentation. The av time between admission and initiation of nutritional support was 11 +/- 15 days SD. Eight pts received a parenteral diet. Two pts received an elemental enteral diet. The av percent change in serum total protein and albumin and nitrogen balance following nutritional support are tabulated. The av wt gain was 1.8 +/- 4.57 kg SD. The av total cost of hospitalization was 54,815 dollars. The av cost of nutritional support was 2917 dollars (5.3% of the total hospital cost). Conclusion: Nutritional support of malnourished AIDS pts results in a wt gain and an increase in serum total protein and albumin and nitrogen balance. The av cost of nutritional support was 5.3% of the total hospital cost. Nutritional support is an underutilized treatment modality in the overall management of AIDS pts.
