Abstract: Since 1982, a suburban, university-affiliated teaching institution has seen 1591 HIV+ patients (pts). We had previously reported (Am J Med 82:389, 1987) our experience with HAM which we are now updating. Demographics are: male 74.6%; white 74.7%. Risk factors: homosexual 38.6%; IVDA 33.9%; heterosexual 15.7%; transfusion 4.8%; other 7.0%. There were 177 (11%) malignancies diagnosed. This included 94 (53%) Kaposi's sarcoma (KS), 63 (36%) hematologic (H), 19 (11%) solid tumors. The 63 H malignancies were non-Hodgkin's lymphomas; 56 (including CNS lymphoma, 17); Hodgkin's disease 5; CLL 1; multiple myeloma 1. One pt has both Hodgkin's and non-Hodgkin's lymphoma. The 19 solid tumors were basal cell 4; testicular 3; head and neck 2; lung 2; thyroid 2; cervix 2; colon, pancreas, breast, gastric, 1 each. There were 15 (78.9%) male; risk factors: homosexual 7; heterosexual 3; transfusion 4; IVDA 5. 11/19 (58%) of the solid tumor pts were greater than or equal to 50 yr of age (mean age: 49.4; median 58) compared to 8.6% of the entire HIV+ population (p less than 0.001) and 13.9% of the combined KS and H populations (mean age 37.6; median 36; p less than 0.001). Transfusional risk was greater in the solid tumor as compared to the entire HIV+ group (0.21 vs 0.048; p=0.009). Solid tumors as a HAM are rare (1.2%) and are more prevalent in the older HIV-infected individual. This probably reflects its incidence in the general HIV- population. Older individuals and their sexual contacts should be questioned regarding all HIV risk factors.
