LUNG CANCER IN HIV-INFECTED PATIENTS (MEETING ABSTRACT)

Abstract: This retrospective study determined the clinical course of lung cancer in patients (pts) with human immunodeficiency virus (HIV) infection. Twenty-four pts with HIV infection archived as lung cancer were studied. Seventeen were identified from about 1000 lung cancer pts entered in the tumor registry and medical records of Jackson Memorial Hospital. Seven more were identified from about 1000 HIV-positive pts entered in the Special Immunology registry of Veteran's Administration Medical Center. Four pts did not have pathologic confirmation of lung cancer and, in one, metastatic esophageal cancer was established. Nineteen pts, all men, met the criteria for analysis (histopathologic diagnosis of lung cancer and HIV+ by serology). The median age (and range) was 48 (36-66) yr. Risk factors for HIV were homosexuality (6 pts), blood transfusion (3), heterosexual promiscuity (5), iv drug abuse (3), and none (3). Six pts had pulmonary tuberculosis and 5 had Pneumocystis carinii pneumonia. Histologic subtypes were adeno: 8, squamous: 6, and others: 5. Lung cancer stages were I: 3, II: 1, III: 5, and IV: 10. CDC stages of HIV infection were I: 0, II: 5, III: 4, and IV: 10. Median survival from advanced stages of both HIV infection and lung cancer may account for the poor survival. All pts with both lung cancer and HIV infection reported so far were men and noted to be younger than other pts with lung cancer.
