Abstract: Lung cancer may be infrequently associated with HIV infection. This retrospective case-control study was undertaken to determine if there were differences in age, sex and stage distribution and in survival between HIV-positive and HIV-indeterminate lung cancer patients (pts). We compared 19 pts with both pathologically proven lung cancer and HIV infection proven by serology diagnosed between 1986 and July 1991 with 1335 pts with lung cancer and indeterminate HIV status diagnosed between 1977-1986. One of HIV+ and the other 1335 pts were extracted from our prior analysis (Am J Clin Oncol, Dec 1991). All 19 HIV-positive lung cancer pts were men. This was significantly (p=0.004) different from the 69% male preponderance in HIV-indeterminate lung cancer pts. Median ages of HIV-positive and HIV-indeterminate pts were 48 and 61 yr, respectively. HIV-positive pts were significantly (p=0.003) younger. Stage distribution was similar in both groups. Survival data which were available in 16 HIV-positive pts were compared with 32 HIV-indeterminate controls matched for stage, age, sex and race. The histologic subtypes between the 2 groups were not different. The median survival in HIV-positive groups was 3 mo and in HIV-indeterminate cohort 10 mo. The survival was significantly different (p=0.002). There were no 1-yr survivors in HIV-positive lung cancer pts.
