CHANGES IN BREAST CANCER TREATMENT MODALITIES IN THE LAST DECADE IN PUERTO RICO (MEETING ABSTRACT)

Abstract:
The incidence and mortality rate of breast cancer in Puerto Rico has tripled in the last decade although both figures are significantly lower than those of the United States. In order to try to identify factors in the treatment modalities that may relate to the low mortality rate, we studied 2 groups of 85 patients (pts) treated in PR in 1983 and 1990, respectively. We compared these groups with respect to (1) frequency of use of medical and surgical modalities, (2) use of radiotherapy, (3) type of surgical and chemotherapeutic modalities used, and (4) incidence of recurrences. A table shows the frequency of surgical approaches used. There was (1) no significant difference in stage (S; S0-I 17%, SII 60%, SIII 15%, and SIV 7%), (2) no difference in the number of pts receiving either a single or a combination therapy, (3) a significant increase in breast-preserving procedures in 1990, (4) no change in the use of MRM and (5) no difference in the type of chemotherapeutic agents or amount of radiation administered. Even when there was a tendency toward a more conservative surgical approach, its use is still lower than in the US probably due to the low number of pts in early stages. There were no other changes in the treatment modalities that can explain the lower mortality rate in PR.
