HODGKIN'S DISEASE (HD) IN PATIENTS (PTS) WITH HIV INFECTION: 85 CASES FROM ITALY (MEETING ABSTRACT)

Abstract:
Since 1986, Gruppo Italiano Cooperativo AIDS e Tumori (GICAT) has been collecting data on malignant tumors in pts with HIV infection. As of March 1991, 85 pts with HD and HIV infection have been observed in Italy. Ninety-two of the pts are males and the median age is 27 yr (range 20-41). Iv drug users (IVDUs) constitute the majority of the cases, 71/85 (73%), while homosexual men are only 9/85 (10%). At the diagnosis of HD, HIV infection was asymptomatic in 27 pts, PGL in 35%, AIDS-related complex in 25% and CDC-defined AIDS in 13% of the pts. The median value of CD4 cells at the diagnosis of HD was 267 mm3 (31-1316). The histological subtypes were: NS in 26%, MC in 51%, LD in 20% and LP in 2% of the cases. Stages III and IV have been detected in 30% and 48% of the pts, respectively, with disease involving extra nodal areas in 59% of the cases. Seventeen percent of the pts have not received any treatment, while of the remaining, 91% received chemotherapy (MOPP +/- ABVD) +/- RT. CR and PR have been obtained in 46% and 52% of the pts respectively. The median survival is 16 mo, mortality was 55% and the opportunistic infections (OI) were the main cause of death (35% alone and 20% in association with HD). A prospective Phase II study is ongoing within the GICAT, with a combined approach with concomitant chemotherapy (EBV: epirubicin, bleomycin, vinblastine; a regimen employed within the EORTC Lymphoma Group) and antiretroviral therapy (zidovudine 500 mg/day per os). As of March 1991, 14 pts have been entered and preliminary results showed that the CR rate was observed in 40% of the cases without any severe toxicity and any progression of HIV infection under therapy. In conclusion, HD in pts with HIV infection is observed prevalently in Italy among IVDUs in agreement with the epidemiology of AIDS, presents in advanced stages with MC, and LD histologies have been observed in the majority of pts. OI are the major cause of death. A prospective ongoing chemotherapy and antiretroviral combined therapy seems promising.
