Abstract:
Long-term survivors of Hodgkin's disease (HD) manifest certain late therapy-related complications. However, certain constitutional features of Hodgkin's patients (pts) may be manifested before any form of diagnostic or therapeutic intervention or they may develop in 'cured' pts without any relation to therapy. Consequently, these effects cannot be fully isolated in treated pts. Premorbid immunocompetence may be important in this context. Immunocompetence in pts with HD is reviewed under the following headings: untreated HD pts (blood lymphocyte subpopulations, blood lymphocyte functions, lymphocytes activated in vivo, clinical correlations of blood lymphocyte counts and functions, and antibody-mediated immunity), some confounding factors in studies of long-term survivors; effects of irradiation on the immune system (thymus irradiation, lymphocyte subpopulations, T-cell functions, nonlymphoid tumors, and non-Hodgkin's lymphoma); immunosuppressive effects of chemotherapeutic agents (lymphocyte subpopulations and functions and diseases other than lymphoma); delayed hypersensitivity, humoral immunity, and inflammatory reactions in HD (complement and phagocytic cells); lymphocytes activated in vivo following curative treatment in HD effects of splenectomy (lymphocyte subpopulations, lymphocyte functions, and serum Igs); effects of immunopotentiating therapy in untreated and treated pts with HD; immune functions of HD pts in remission following successful treatment; implications and consequences of immune deficit in disease-free Hodgkin's pts (pre- and post-treatment immune status in relation to prognosis, immunodeficiency and increased risk of second malignancies, and post-treatment immunodeficiency susceptibility to infection); mechanisms of the persistent T-cell defect in HD, lymphocyte maldistribution as a tentative mechanistic model of immunodeficiency in untreated and treated HD pts; immunologic studies in family members of pts with HD; immunologic impairment and the pathogenesis of HD; and a hypothesis relating immunodeficiency to etiology, tumor, and treatment.
