Abstract: The oxidant stress may alter cell immunity and has been associated with lung cancer. A premedication with antioxidants protects from the experimental tumor. The reduction of alveolar macrophage activity and CD4/CD8 lymphocyte ratio in bronchoalveolar lavage (BAL) is related to lung cancer and smoking (an important source of oxidants). To check the activity of antioxidants on cell immunity of the alveolar environment before and after administering aerosol-reduced glutathione (600 mg bid for 6 days), we evaluated the CD3, CD4, CD8 lymphocyte subsets and CD4/CD8 ratio in BAL (50 ml of normal saline solution at 37 C for 4x in the middle lobe) and alveolar macrophage phagocytosis (superoxide anion production after phagocytosis of opsonized zymosan). The 10 patients studied (7 males; 3 females) showed an increase in CD4/CD8 ratio (which nevertheless does not reach any statistical significance) and an improvement of alveolar macrophage phagocytosis (33.2 vs 50.7; p less than 0.04). After administering glutathione in an aerosol, our data show immunological changes opposite to those associated with lung cancer, and also might indicate that the protection from lung cancer provided by antioxidant compounds could be produced by improved immune surveillance.
