Abstract: The most important cytokines acting on human B cells seem to be interleukin 4 (IL-4), IL-2, and IL-6. The availability of cloned and purified B cell-tropic cytokines has opened the possibility of investigating their role in the pathogenesis of some human diseases and testing this knowledge in vivo. The role of B cell-tropic cytokines in disorders involving immunodeficiency, autoimmunity, and allergy is reviewed. Topics include common variable hypogammaglobulinemia (CVH), AIDS, systemic lupus erythematosus (SLE), rheumatoid arthritis (RA), other autoimmune disorders, pathological conditions characterized by hyperproduction of IgE, IgE disruption in AIDS and Hodgkin's disease, vernal conjunctivitis, parasitic infestations, and atopic diseases. At least in some patients (pts) with CVH, impaired function of, and/or response to, cytokines may be responsible for the immune defects. Increased production of IL-6 by HIV-infected macrophages may contribute to the enhanced antibody formation in pts with AIDS. Excessive IL-6 production also may account for the local, and several of the generalized, symptoms in pts with RA, including fever, production of acute phase proteins, and hypergammaglobulinemia. B-cell hyperactivity leading to enhanced antibody and autoantibody formation in pts with SLE and other systemic and organ-specific autoimmune disorders probably is associated with states of hyperfunction of some cytokine/cytokine receptor systems, such as IL-2, IL-4, IL-5, and IL-6. However, understanding of the role and degree of cytokine alterations in such diseases still is very poor. More clear and detailed evidence suggests a role for cytokine alterations in allergy and in other pathological conditions characterized by hyperproduction of IgE. A detailed analysis of the therapeutic potential of B cell-tropic cytokines now is a major research priority. Not only may the cytokines be useful in vivo, but antibodies against them or their receptors may have therapeutic potential is some immunodeficiency syndromes, autoimmune diseases, and hypersensitivity states.
