CLINICAL APPLICATIONS OF ERYTHROPOIETIN (MEETING ABSTRACT)

Abstract:
Since its introduction to the clinic in 1985, recombinant human erythropoietin has been used in a diverse range of physiologic replacement applications and pharmacologic interventions in the anemic patient (pt) or the surgical subject. The first anemia of renal failure studies conducted in Washington and in England showed remarkably precise dose response curves, with time to response very tightly correlated with dose administered, on a 3x/wk schedule. This schedule was chosen because of convenience to the dialysis pt, but has proven to be a remarkably robust schedule across all indications. Studies in the AIDS population, while not as consistently positive as in renal failure, have shown that in the face of moderately elevated erythropoietin levels, pharmacologic intervention with erythropoietin can correct anemia. The cancer pt falls in an intermediate setting between the 'physiologic replacement' of renal failure pts and the 'pharmacologic intervention' of AIDS pts. Data support that in all cancer populations erythropoietin can be safely administered to pts as long as caution is maintained around the rate of rise of hematocrit and careful monitoring of blood pressure. Interventions in relatively normal subjects about to undergo surgery have shown that erythropoietin can augment the available amount of autologous blood at the time of surgery, through both predeposit storage and increasing the hematocrit more rapidly postoperatively. Data from the series of studies conducted by the RW Johnson Pharmaceutical Research Institute in these settings are presented.
