Abstract: The CT and MRI morphology of 30 cases with CNS lymphomas in AIDS patients was reviewed and compared with histological findings. Surprisingly, CT and MRI showed a bright spectrum of appearances, ranging from solid, nodular tumors to necrotic masses and very irregular types of spreading, which can mimic glioblastoma or encephalitis. Metastatic spreading along the CSF spaces could be proven by gadolinium-enhanced MRI in 3 cases. The different types of tumor morphology found by imaging modalities showed a good correspondence to histological findings, where regular patterns of tumor growth could be distinguished from 'wild' and infiltrating types of spreading. The invasion of the meninges and the CSF spaces could also be confirmed by neuropathology.
