CLINICAL INSIGHTS IN HIV-RELATED MALIGNANCIES (MEETING ABSTRACT)

Abstract:
HIV infection not only produces severe immunodeficiency, but also has produced an epidemic of lymphoma and Kaposi's sarcoma as well as other neoplastic conditions. We have been following 1530 HIV-infected patients (pts). We have recorded the appearance of the following neoplastic conditions: Kaposi's sarcoma (KS) +/- opportunistic infection, non-Hodgkin's lymphoma (NHL), CNS lymphoma, Hodgkin's disease, chronic lymphatic leukemia, and cervical invasive neoplasia (CIN). Incidence by risk group is shown in a table. In an ongoing study of CMV viremia complicating HIV, pts with KS have an inordinately high incidence of CMV viremia, particularly in advanced KS (almost 70%) in contrast to a surprisingly low level of CMV viremia in pts with concomitant lymphoma (less than 20%). KS pts also develop more CMV organ-specific complications. CNS lymphoma and NHL occurred in all groups, occurring with highest frequency in transfusion pts. The true rate of lymphoma in our pts is underestimated as many of them have early HIV infection; the true rate of lymphoma is probably 3x higher by the time a pt dies of HIV infection. Tumor incidence and characteristics are summarized. Some solid tumors occurred in our pts but only testicular cancer appears to be occurring in a higher-than-expected frequency. The incidence of cervical cancer is underestimated, as many women fail to obtain adequate gynecologic care. The incidence of CIN is high in pts who do not have PAP tests. HIV has produced a major associated epidemic of cancer. The infectious agent causing KS and NHL still needs to be defined. The epidemiology of KS clearly indicates that it is caused by an infectious agent.
