Cervical intraepithelial neoplasia in HIV-infected women.

Abstract:
OBJECTIVES: To compare, in HIV-infected and HIV-negative women, 1) the characteristics of cervical intraepithelial neoplasia (CIN) and (2) the course of CIN after treatment. METHODS: (1) Women with Pap smears of CIN1 or worse attending a colposcopy service were counselled and tested for HIV. Lesion extension was determined by colposcopy and CIN severity by biopsy. (2) HIV-seropositive and HIV-seronegative CIN patients were treated by standard ablative therapy and followed with at least one Pap smear. Persistence/recurrence was defined as biopsy-proven CIN and rates were calculated by the product-limit method. RESULTS: (1) Compared to 132 HIV-seronegative CIN patients, 52 HIV-seropositive CIN patients were older, had more severe CIN (p less than 0.01), more extensive cervical lesions (p less than 0.05) and more associated vulvovaginal lesions (p less than 0.1). In HIV-seropositive women, CIN was more severe after adjustment for age. (2) CIN persisted or recurred within one year in 40% of 33 HIV-positive CIN patients compared to 7% of 109 HIV-negative CIN patients (p less than 0.05). Recurrence rate was not related to age or CIN severity. CONCLUSIONS: In patients with equivalent cytology, CIN is likely to be more extensive and of higher grade in the HIV-infected. Since CIN is more likely to recur, HIV-infected women must be closely monitored by colposcopy and new therapeutic strategies developed. TABULAR DATA, SEE ABSTRACT VOLUME.
