Study of the behaviour of HIV positive or negative IVDUs in their family circle.

Abstract:
DESCRIPTION OF PROJECT: The Centre Cassini, which is the second structure of the Assistance Publique-Hopitaux de Paris was opened on October 1st, 1990. The centre is linked to Dr Peigne's psychiatry department at the hospital Cochin, but it is located outside the hospital. It favours the welcoming, informing directing and follow up of IVDUs (Intravenous drug users) and theirs families. METHODS: The centre provides: 1. a psychotherapeutic support and outpatient care, 2. a social and psycho-educational follow-up, 3. medical consultations that cover internal medicine, STD, HIV, dermatology, gynaecology and family planning, 4. the directing of patients towards medical services or specialized centres for aftercare and drug eradication, 5. training for the AP-HP staff. RESULTS: A year and a half's functioning thus shows that: 439 patients have benefited from a psychotherapeutic, socio-educational and medical support. Among them, 59 came with their family to consult either just once or regularly. TABULAR DATA, SEE ABSTRACT VOLUME. This proved the necessity of a global support. We were able to make the following observations. HIV positive, IVDUs have a personal approach concerning drug eradication and psychotherapeutic and medical support regarding HIV. Their being HIV positive does not affect their sexual behaviour therefore our prevention scheme may be called into doubt. On the other hand, HIV negative IVDUs implicitly ask for it (often times it is expressed by one of the family members). Besides all parents of HIV negative IVDUs have this attitude in common which consists in trying to act in a compensatory manner and in overprotecting their children. This leads to an interactive behaviour regarded as immature. LESSONS LEARNED: The families of IVDUs turn to care centers at critical times, when the state of the IVDU deteriorates. This leads us to the conclusion that the family's behaviour is similar to that of the IVDU's as both of them ask for help at a dangerous time which is also one of nervous tension, within the family circle because of HIV positivity.
