Abstract: R. equi, an emerging pathogen in severely immunocompromised patients, has recently been reported to cause disseminated fatal infections in HIV-infected patients. However, the diagnosis may be very difficult; definitive microbiologic identificatio may take 2 weeks and no fluorescein-labeled antibody test exists for R. equi. Histopathologic examination of infected tissues with conventional stains may be unhelpful because R. equi may be present in small numbers and/or have nonspecific morphology (coccoid forms). We studied paraffin-embedded infected tissues from 3 HIV-infected patients with fatal R. equi infections using ISH with a biotin-labeled DNA probe. Intracytoplasmic R. equi microorganisms were distributed mainly in macrophages. Control tissues from patients with unrelated infections were negative for R. equi infection. These data suggest that this specific nonisotopic test may provide a useful and rapid tool to diagnose R. equi infection. Thus, patients at risk for life-threatening R. equi infections may have an earlier diagnosis and an improved prognosis.
