Abstract: Newly synthesized rifamycin derivatives, benzoxazinorifamycins (KRMs; Kanegafuchi Chemical Ind. Co., Osaka, Japan), have a markedly improved in vitro activity against various mycobacteria including M.avium complex (MAC), and their MICs against most strains were more than 64 times lower than those of RFP. In this study, we examined in vivo activity of the KRMs against MAC infections induced in mice and rabbits. When BALB/c mice infected iv with M.intracellulare N-260 strain were given each of five KRMs suspended in arabic gum-Tween 80 solution by lavage at doses of 0.2 and 0.4 mg/mouse, daily, 6 times per week, from day 1, for up to 8 weeks after infection, the incidence of gross lung lesions was completely inhibited, accompanied by 2-3 order decrease in the number of CFU of the organisms in the lungs, as compared to untreated animals. When NZW rabbits infected iv with M.avium N-425 strain were given KRM-1648, which exhibited most superior efficacy in the murine experimental infections, at doses of 25 and 50 mg/Kg in the same protocol as in the case of mice, extension of the survival time and 1-3 order decrease in the number of CFU of the organisms in the lungs, liver, spleen and kidneys were noted. Further, MAC-induced bactermia, as seen in MAC-associated AIDS patients, was also nearly completely suppressed by the KRM-treatment.
