Abstract: Aim of the research has been to define a molecular boundary between virus latency and active replication phase in course of HIV-1 infection. For this purpose PBLs and plasma from seropositive subjects were checked for the presence of HIV-1 specific transcipts and genomic RNA (HIV-1 viremia), respectively, using reverse transcription-polymerase chain reaction technique to amplify highly conserved sequences of gag and env ORFs. A high proportion (21/33; 64%) of seropositive subjects with more than 400/microliter CD4+ lymphocytes, as well as 25/28 (89%) immunodeficient patients (CD4+ lower than 400/microliter) show specific viral transcripts. The high majority (28/30; 93%) of subjects investigated for the presence of genomic HIV-1 RNA is viremic. These results indicate that: 1) detection of HIV-1 transcripts in PBLs of seropositive subjects during clinical latency allows the very early identification of patients requiring specific antiviral therapy; 2) a basal HIV-1 replication level is detectable as viremia in most infected subjects.
