Comparison of whole cell viral lysate (VL), synthetic peptide (SP), and recombinant protein (RCP) EIA's for detection of HIV-1 antibody.

Abstract:
Five hundred seventy five serum specimens received for HIV-1 antibody determination were tested prospectively by EIA using a whole cell viral lysate (Genetic Systems Corp.) a synthetic peptide (United Biomedical, Inc.) and a recombinant protein (Syva Co.). Concordance of all 3 antigens was noted for 558 of 575 (97%) specimens tested. Ninety (15.7%) specimens were positive and 468 (84.3%) were negative. Of the 16 discrepant results, 8 were non-specifically positive by VL and 7 were non-specifically positive by SP. The RCP ntigen gave no non-specific positive reactions, however there was one false-negative result. Retrospective testing by SP and RCP of 86 specimens indeterminant for HIV-1 antibody by VL, Western blot, and immunofluorescence also was performed. All 3 EIA screening methods agreed for 29 (33.7%) specimens. Forty five specimens were reproducibly reactive (rr) only by VL, 9 were rr by VL and SP, 2 were rr only by SP, and 1 was rr by VL and RCP. None of the 86 specimens were rr only by RCP. The results of both phases of this study indicate the specificity of the 3 antigens is RCP SP VL. As noted from the prospective phase, the sensitivity of the antigens appears to be equivalent.
