Abstract: Three commercial synthetic peptide EIAs (PEIA) for HIV-1 and -2 antibody and a combined HIV-1/2 EIA were evaluated for their sensitivity and specificity as compared to FDA-licensed whole virus lysate EIAs (VEIA) and Western blots (WB). A panel of 107 HIV-1 weakly reactive sera, 19 HIV-1 seroconversion panels, and 158 HIV-2 sera were tested. The PEIA-1 and -2 found to be most sensitive and specific were used in an screening algorithm to test 1019 IVDUs for HIV-1 and -2. Results were compared to a standard testing algorithms (VEIA followed by WB.) The sensitivity of the PEIAs for HIV-1 ranged from 89.1% to 99.2%. Genetic Systems PEIA-1 and -2 were the most sensitive and specific assays. A screening algorithm including PEIAs for HIV-1 and -2 identified all infected specimens, slightly reduced the number of WB indeterminate results, but was more expensive than VEIA + WB.
