Abstract: Epidemic Kaposi's sarcoma (EKS) is a common manifestation of AIDS, and frequently involves multiple, widespread cutaneous sites. Although limited-field radiotherapy has been useful in the management of EKS, the role of total skin electron beam (TSEB) irradiation has never been adequately assessed. Since August 1988, 13 patients (pts) with EKS have undergone TSEB irradiation at our institution. Pts were treated with linear accelerator-generated electron beams of 4 and/or 5 MeV energies utilizing either 4- or 8-field technique. Doses employed ranged from 300-2400 cGy (median 1500 cGy) delivered in 1-8 fractions. Follow-up ranged from 1 to 23 mo (median 9 mo). Of the 12 pts evaluable post-treatment, 10 experienced a significant response to TSEB irradiation, with 5 pts experiencing total regression of all lesions. The remaining 2 pts exhibited stabilization of their EKS. Ten of the 13 pts treated have died, with post-treatment survival ranging from 2-23 mo (median 9 mo). The single recurrence noted occurred 5 mo following a complete response to 600 cGy. TSEB irradiation was well tolerated, in that 8 pts experienced no significant treatment-related toxicity, 3 experienced dry desquamation and 2 experienced moist desquamation. We conclude that TSEB irradiation is a safe and effective treatment for cutaneous EKS.
