DRONABINOL ENHANCES APPETITE AND CONTROLS WEIGHT LOSS IN HIV PATIENTS (MEETING ABSTRACT)

Abstract:
Dronabinol, delta-9-THC in sesame oil (Marinol, Roxane Laboratories, Columbus, OH) has been noted in a past study to cause wt gain in HIV patients (pts). To date 19 symptomatic HIV pts with a history of loss of greater than 5% total body wt over 3 mo prior to study entry have been treated. This is an open, dose-ranging study; pt accrual is continuing. After a 1-wk observation period, pts received dronabinol daily for 4 wk. Appetite was measured before each meal and mood once daily on a 100 mm visual analog scale. Pts were considered evaluable if they received at least 2 wk of dronabinol. Three pts (1 each at 2.5 bid, 2.5 tid and 5 bid) discontinued due to intercurrent illness; 5 (4 at 2.5 tid and 1 at 5 bid) due to moderate but persistent high, and 1 each (2.5 tid) due to moderate paranoid feelings and severe vomiting possibly related to study medication; side effects subsided within hours upon discontinuation of treatment. Results are presented in a table. Dosing bid (before lunch and dinner) was well tolerated. Of 11 evaluable pts, 4 gained wt and 6 lost wt less rapidly when on dronabinol than before treatment; all but 2 pts noted improvement in appetite and mood.
