Abstract: Ten patients with AIDS-associated NHL were treated with 2-CDA in a Phase I dose escalation study. All subjects had histologic diagnosis of high grade B-cell lymphoma, including small noncleaved (3), immunoblastic (3), large cell (2), Burkitt's (1), and unclassified (1). All subjects had Stage IV disease. Nine subjects relapsed after prior chemotherapy and one was previously untreated. Five subjects had an antecedent AIDS diagnosis including 2 with Kaposi's sarcoma (KS), 2 with P carinii pneumonia (PCP), and one with both KS and PCP. The mean CD4+ cell count was 0.049 x 10(9)/L. 2-CDA was administered by 7-day continuous infusion; patients received from 1 to 3 cycles of 2-CDA. Two subjects achieved partial responses and one subject achieved a minimal response. All subjects died with progressive disease. Results by dose level are shown in a table. Toxicity was minimal. Myelosuppression developed in one subject with a transient grade 4 neutropenia that resolved with stopping the infusion. Low-grade fevers were common and superficial phlebitis was observed at the site of infusion. There were no CNS or renal toxicities noted. 2-CDA has activity against AIDS-associated NHL in subjects with refractory disease. It is well tolerated with minimal side effects. The unique mechanism of action of the drug combined with its low toxicity makes it an appealing drug to add to standard chemotherapy for this disease.
