Abstract:
Advances in recombinant DNA technology leading to the development, manufacture, and establishment of the clinical efficacy and safety of naturally occurring human proteins, are having a profound impact on the day-to-day practice of medicine. The availability of recombinant human erythropoietin, a blood growth factor, marks yet another new horizon. The use of erythropoietin has substantially and beneficially changed the lives of thousands of patients (pts) with anemia secondary to chronic renal failure. The potential for erythropoietin to result in meaningful therapeutic advances in an even broader range of diseases is realistic. Expectations are that pts worldwide will benefit from the therapeutic administration of erythropoietin for diverse indications, including cancer chemotherapy and AIDS, as well as from its prophylactic use in autologous blood storage. The clinical use of erythropoietin is reviewed, including its history, molecular biology, biochemistry, and biology; the pathogenesis of anemia secondary to chronic renal failure; treatment of pts with renal anemia with recombinant human erythropoietin (European experience); erythropoietin treatment of pts with end-stage renal disease (North American and Japanese experience); the use of erythropoietin in predialysis pts; quality of life issues with erythropoietin; potential use of erythropoietin in the pediatric population; erythropoietin in the surgical setting; and erythropoietin therapy for the anemia associated with AIDS, AIDS therapy, and cancer.
