Abstract: The majority of non-Hodgkin's lymphomas in AIDS are B-cell in origin. We describe a CD4+ T-cell lymphoma that expresses HIV-1 p24 antigen and appears to be the first case described with clonally integrated HIV sequence. Such integration may indicate a direct transforming role of the virus. Multiple sites of tissue were isolated at autopsy from a 45-yr-old man who died of a fulminant large-cell lymphoma. The immunophenotype of the lung tumor was LCA+, CD4+, CD25+, CD8-, CD3-. Tumor cells expressed HIV-1 p24. Southern blot analysis for rearrangement of the T-cell receptor gene (C[T]beta) indicated a monoclonal pattern. DNA probed with a 3.5-kb 3'-HIV fragment digested with BamH1 showed two distinct bands at 6 kb but only a single 9.6-kb band on XbaI digest, as well as a larger fragment with undigested DNA. The presence of two distinct bands in the BamH1 digest but a single fragment on XbaI, suggest integration of HIV-1 (internal BamH1 site with flanking XbaI sites). Probes for HTLV-1 genes were negative. Further gene mapping and sequencing will determine if there are structural analogies to transforming retroviruses described in animals.
