Comparison of three ELISA kits for the determination of antibodies to HIV-1.

Abstract: The performance of 3 enzyme-linked immunoassay (ELISA) kits, each using different antigens and principles, were compared for their relative ability to detect antibodies to Human Immunodeficiency Virus type 1 (HIV-1). The kits selected were Genetics (GEN), Wellcozyme (WEL) and I.A.F. BioChem (IAFB). A total of 350 samples from different risk groups were tested separately by each kit. Some of these samples previously gave ambiguous results by one or more ELISA kits used in routine testing. Any ELISA reactive sample was verified by Immunofluorescence assay, Western blot (WB) or Radioimmune precipitation assay (RIPA). Of 350 samples, 72 were verified as true positives by these reference tests. GEN identified 68 of 72 positives (sensitivity 94.4%; specificity 82.2%). WEL identified 63 of 73, (sensitivity 87.5%; specificity 86.7%). IFAB gave the best results and identified 71 of 72 positives (sensitivity 98.6%; specificity 97.8%). All kits did not identify an early seroconversion with a trace gp 120/160 band by WB (also RIPA positive). In addition, the IAFB kit performed the best on a panel of diluted serum samples sent by the College of American Pathologists Laboratory survey. These results show that the IAFB kit which uses a synthesized antigen equivalent to conservatively retained structural HIV-1 proteins, offers an improved performance when tested against this panel of sera.
