TUMOR MARKERS IN DIAGNOSIS AND SCREENING

Abstract:
A wide variety of substances can be identified as tumor markers. These include tumor-associated antigens, enzymes, specific proteins, and metabolites. Tumor markers employed in screening for cancer in general and in diagnosis and management of germ cell tumors and ovarian, colorectal, lung, breast, and prostate cancers are reviewed. Among the tumor markers and putative tumor markers considered are alpha-fetoprotein, carcinoembryonic antigen, vanillylmandelic acid, chorionic gonadotropin, CA-125, CA-19-9, ferritin, sialic acids, neuron specific enolase, CA-15-3, phosphohexose isomerase, and prostate-specific antigen. Measurement of tumor markers has been suggested for screening both total populations and high-risk groups. Markers are used in diagnosis either as aids in staging or in confirmation of histopathology. Tumor markers also have been used in therapy to predict drug response and prognosis and to follow the course of the disease. Circulating tumor markers have an important role in the management of patients with breast, lung, ovary, and prostate cancers. The need for the future is to identify patients who are at high risk for cancer. Analysis of oncogenes may play this role when the technology becomes more suitable for mass clinical application.
