Abstract:
As late as 1979, antiviral agents were somewhat of a novelty in clinical medicine. Their systematic discovery and development had not progressed much beyond research applications. Because of the research efforts stimulated in response to the AIDS epidemic, new antiviral agents have been developed recently. These include azidothymidine against AIDS, ribavirin for respiratory syncytial infections, dihydroxypropoxymethylguanine for ocular cytomegalovirus (CMV) infection, interferon (IFN), and rimantadine for influenza. The basic biology and clinical applications of antiviral agents are reviewed, including virus replication (target functions and events for virus-specific inhibitors); pathogenesis of viral infections; preclinical evaluation of antiviral agents (in vitro and animal model testing); clinical drug development (human pharmacology, safety, and tolerance trials); laboratory diagnosis of viral infections; major ocular viral infections; varicella-zoster virus infections; herpes simplex virus; human papillomaviruses and IFN therapy; respiratory diseases; viral infections of the gastrointestinal tract; antiviral therapy of viral hepatitis; viral infections of the CNS; chronic intrauterine and perinatal infections; HIV and other human retroviruses; Epstein-Barr virus and human herpesvirus type 6; CMV and the immunosuppressed patient; RNA viruses that cause hemorrhagic, encephalitic, and febrile disease; and immunization against viral diseases.
