NOVEL COMBINATION CHEMOTHERAPY FOR HODGKIN'S DISEASE (HD) IN HIV-INFECTED INDIVIDUALS (MEETING ABSTRACT)

Abstract:
Five previously untreated HIV-seropositive men with histologically diagnosed HD were treated with bleomycin 10 U/m2 iv day 1; vincristine 1.4 mg/m2 iv day 1; streptozocin 750 mg/m2 iv day 1, 3, 5; and etoposide 100 mg/m2 iv day 1, 3, 5. Aerosolized pentamidine, 300 mg/mo was administered for P carinii prophylaxis. HIV risk group included homosexual (3), iv drug user (1), homosexual iv drug user (1). Mean age was 33 and no patient (pt) had a prior CDC-defined AIDS diagnosis. The median pretreatment CD4-positive lymphocyte count was 278/mm3 (range, 120-2110). Histologic pattern was nodular sclerosis in 3 and mixed cellularity in 2. Pretreatment staging with CT and bone marrow biopsy revealed Stage III disease in 2 and Stage IV in 3. Extranodal sites included bone marrow (1), liver (1), bone marrow and liver (1). Twenty-eight of a planned 30 cycles of chemotherapy were administered, all at full dose. The mean nadir neutrophil count for the group was 2050/mm3 (range 1.15-3.52). No cycle was complicated by fever and neutropenia. Toxicity included mild to moderate nausea in 4, transient severe peripheral neuropathy in 1, and transient moderate bilirubin elevation in 1. A self-limited episode of cryptosporidium diarrhea occurred in 1 pt. No other opportunistic infections occurred during therapy. Complete response occurred in 4 (80%) and partial response in 1 (20%). All remain alive at 5, 5, 6, 15, and 19 mo from diagnosis. These early results compare favorably with the poor response rates, short-survival times, severe myelosuppression and less than 50% dose intensity previously reported for similar pts receiving standard HD chemotherapy. This trial is ongoing.
