Abstract:
Considerable advances have been made against several types of cancer, with large improvements in long-term survival for acute lymphocytic leukemia, Hodgkin's disease, diffuse lymphoma, testicular cancer, choriocarcinoma, childhood sarcomas, postmenopausal Stage II breast cancer, and small cell lung cancer (limited stage). Despite this, numerous patients in the United States do not receive the best treatment available. The NCI has initiated several programs to transfer treatment research advances to the community (eg, Physician Data Query Computer Data Base, Cooperative Group Outreach Programs, and Office of Communications Cancer 'Hot Line'). Preclinical and clinical research activities sponsored by NCI are reviewed, including drug development (application of the advances in tumor biology); new drug screening programs; AIDS drug development; studies of growth factor antagonists, metastasis blockers, tumor necrosis factor, and immune stimulation therapy/adoptive immunotherapy; and advances in diagnostic imaging and radiation therapy. NCI-supported clinical treatment trials are bringing the advances in basic cancer biology into the arena of clinical cancer medicine. Over 5000 physicians engaged in NCI studies follow 75,000 patients annually and see an additional 200,000 patients with cancer each year. The NCI is unique within NIH both in developing new anticancer drugs using the contract mechanism and in its support of a nationwide network of cancer centers, outreach programs, community clinical oncology programs, and cooperative groups for clinical trials.
