Abstract:
Non-Hodgkin's lymphoma (NHL) and Hodgkin's disease (HD) are described in patients (pts) with HIV infection. The etiology of NHL in this population remains unclear. The most prominent clinical feature of NHL in the setting of HIV infection is the presence of extralymphatic disease, and NHL should always be considered in the differential diagnosis of HIV-seropositive pts with unexplained wasting syndrome or chronic fevers, anemia, neutropenia, rapidly progressive lymphadenopathy, an enlarging mass lesion at any site, gastrointestinal complaints, or CNS abnormalities. Treatment of NHL in the pt with HIV is difficult because aggressive combination chemotherapy may likely exacerbate the pt's immunodeficient state. A number of therapy regimens are discussed, and the choice of therapy must be based on the nature of the disease and the overall condition of the pt. The precise relationship of HD to the underlying immunodeficient state in pts with HIV infection remains unclear. Clinical observations suggest that HD in this setting may have a different natural history and therapeutic outcome when compared with HD in the general population. Pts with HIV infection and HD are likely to have a poor therapeutic outcome and to develop AIDS-associated opportunistic infections during therapy. The use of less intensive and less immunosuppressive chemotherapeutic regimens or combinations of therapy with growth factors should be explored in the treatment of these pts, and antibiotic prophylaxis for Pneumocystis carinii pneumonia should be part of any treatment regimen.
