SPECTRUM OF HTLV-III INFECTION

Abstract: Diseases related to human T-lymphotropic virus/lymphadenopathy-associated virus (HTLV-III/LAV) other than full-blown AIDS are discussed. The spectrum of disease consequent to retroviral infection includes AIDS-related complex (ARC), including lymphadenopathy, herpes zoster and oral candidiasis infection, diarrhea, hematological manifestations; nervous system disease associated with HTLV-III (dementia, other focal neurological disorders, and psychiatric disorders); acute HTLV-III-induced illness (eg, fever, headache, myalgias, photophobia, meningismus, lymphadenopathy); and neoplasms associated with HTLV-III infection (Kaposi's sarcoma, CNS lymphoma, Hodgkin's disease, and other unexpected neoplasms, such as squamous carcinoma of the head and neck, adenocarcinoma, small cell carcinoma of the pancreas, and aggressive seminoma). The clinical manifestations of HTLV-III infection go well beyond that of AIDS as defined by the Centers for Disease Control for purposes of epidemiological surveillance. A wide variety of disorders appear to be the result of infection with this new retrovirus. The retrovirus is not only lymphotropic, but also neurotropic; one of the most prominent of its clinical manifestations is central and peripheral nervous system dysfunction. A number of neoplasms beyond Kaposi's sarcoma and CNS lymphoma arise in the host consequent to HTLV-III infection. (21 Refs)
