Abstract: The precise nature of AIDS-related lymphomas is discussed based on the authors' experience treating 44 men (42 homosexual or bisexual) with malignant lymphoma. An evaluation of the clinical, pathological, immunological, and virological features of the disease was conducted in these patients (pts) to elucidate the factors that serve to define the AIDS-related lymphoma. The malignant lymphomas associated with AIDS are quite distinctive, pathologically and clinically. They may be defined best as high-grade B-cell lymphomas that present in disseminated fashion in unusual extranodal sites such as the CNS or rectum. They are associated with the reversal in the T4:T8 ratio in the peripheral blood, as well as exposure to human T-lymphotropic virus-III/lymphadenopathy-associated virus and distant exposure to the Epstein-Barr virus. Results of treatment using standard intensive multiagent chemotherapy have been disappointing, with median survival of less than 1 yr in treated pts. CNS relapse may occur despite attempts at prophylactic therapy. The precise mechanism of development of lymphoma in these pts remains to be defined. (33 Refs)
