Abstract: In an attempt to identify a neurotropic subtype of HIV, the biologic and serologic properties of seven isolates from the CNS of individuals with and without neurologic symptoms were determined and compared to eight isolates from peripheral blood and lymph nodes. Distinct biologic and serologic properties of HIV isolates from the brain are described, and paired CNS and lymphocyte isolates are compared. HIV isolates from the brain can be distinguished from peripheral blood isolates by their (1) inability to infect established human T-cell lines, (2) relative sensitivity to serum neutralization, and (3) efficient replication in primary macrophages. The results support the hypothesis that HIV infection of the brain results from virus transport to this tissue by peripheral monocytes. Whether a specific cell type in the brain (glial or neurons) is particularly sensitive to HIV infection requires further investigation. In individuals with neurologic disorders, the presence of HIV variants with different biologic properties is likely. The present data suggest that isolates from the CNS in these individuals will be particularly tropic for macrophages and replicate less well in established human T-cell lines. (9 Refs)
