ALGORITHMS FOR THE TREATMENT OF AIDS PATIENTS WITH NEUROLOGICAL DISEASES

Abstract:
Methods of evaluating and treating patients (pts) with AIDS who develop neurologic problems have been in a continuous state of evolution. As new diseases are diagnosed and new treatments are made available, algorithms for clinical decision making are bound to change. Management of pts with neurological complications related to infection with HIV, as it is conducted at the University of California, San Francisco, is described. Among the conditions found in these pts that require management are chronic inflammatory demyelinating polyradiculoneuropathy, mononeuropathy multiplex, distal symmetrical peripheral neuropathy, cytomegalovirus and herpes simplex virus infection, mycobacterial, coccidioid or toxoplasma infections, hydrocephalus, mass lesions and space occupying lesions that do not produce a mass effect, and stupor-coma. Algorithms (flow charts showing decisions and alternatives) are presented for the evaluation and treatment of peripheral nervous system dysfunction, cranial neuropathy, intracerebral or brain stem symptoms, and intracranial mass lesions. Treatments discussed include radiotherapy, chemotherapy (ganciclovir, acyclovir, amphotericin B, 5-flucytosine, azidothymidine), antibiotic therapy (sulfadiazene and pyrimethamine), and open biopsy with decompression.
